Abstract

INTRODUCTION: Pancreatic actinomycosis is an extremely rare condition with few reports. It
is characterized as a chronic granulomatous infection, identified as a malignant-appearing mass
that can lead to unnecessary treatment. Diagnostic suspicion is established through imaging,
and definitive diagnosis is made with histopathology through the visualization of bacterial colonies.
Long-term antibiotic treatment is sufficient for the management of this infection.

CLINICAL CASE: We present the case of a 79-year-old male patient with a history of multiple
pancreatic instrumentalization. A pancreatic tumor was initially suspected, and a needle-guided
biopsy identified Actinomyces colonies. The patient was successfully treated with an antibiotic
regimen.

CONCLUSIONS:
Successful treatment of pancreatic actinomycosis depends on suspecting
the presence of this pathogen in patients with prior surgical instrumentation to make a timely
diagnosis.
