Abstract

Patients diagnosed with COPD admitted in ICU very often require invasive mechanical ventilation which predisposes an increase in overall mortality.
Objective: to describe the mortality of patients diagnosed with COPD requiring invasive mechanical ventilation. Secondary objectives were to identify prognostic factors associated with
mortality.

Methods: retrospective descriptive study of patients with known or suspected diagnosis of COPD admitted to the ICU in the period between 2011 and 2012, requiring invasive mechanical ventilation. A logistic regression analysis was conducted to establish possible variables related with mortality.
Results: a total of 98 patients with suspected or diagnosis with COPD admitted to ICU of Hospital Carlos Andrade Marin, 88 (90%) required use of VMI. The Mortality was 73%, 83% male and 17% female. The average age was 76 years.
Average days of hospitalization in ICU were 7.59 days, and 5.79 days on mechanical ventilation respectebly. The APACHE II score average was 22. The most common cause of COPD for patients
entering the ICU was pulmonary infection. Logistic regression analysis showed association between mortality and invasive mechanical ventilation. OR: 5.8.
Discussion: COPD has been established as one of the leading causes of morbidity and mortality, and the presence of decompensation causes severe respiratory failure requiring admission to the intensive care unit and invasive mechanical ventilation increasing mortality significantly.
